OBSERVED FLOOR TILES MISSING IN THE BREAK ROOM, DRY STORAGE ROOM AND THE ROOM WITH THE WALK-IN COOLER AND FREEZER. MUST REPLACE MISSING FLOOR TILES. DETAIL CLEAN THE FLOOR BEHIND THE STOVE (LITTER AND DEBRIS).

      • The walls and ceilings shall be in good repair and easily cleaned. CAULK THE WALL BEHIND THE RESTROOM HANDSINKS. REPAIR THE LOOSE BASEBOARD IN THE 2ND FLOOR NURSE'S RESTROOM. REPAIR THE OPENINGS IN THE WALL OF THE BREAK ROOM.

      • Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. LIGHT SHIELD IS MISSING FROM THE WALK-IN FREEZER. MUST PROVIDE.

OBSERVED GARBAGE AND LITTER AROUND THE CONSTRUCTION AND FACILITY DUMPSTERS. INSTRUCTED MANAGEMENT TO CLEAN AND MAINTAIN THESE AREAS.
