      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OUTER OPENINGS NOT PROTECTED. OBSERVED REAR EXIT DOOR WITH A 2"GAP ON SIDE AND BOTTOM OF THE DOOR. DOOR AND FRAME ARE RUSTED. ALSO OBSERVED THE DOORS IN DINING ROOM THAT CONNECTS THE PATIO ARE EXTREMELY RUSTED AND THE TOP OF THE FRAME WITH HOLES LARGE ENOUGH FOR INSECT ENTRY. WHEN YOU OPEN THE SAID DOORS RUST FALLS ON YOU AND FLOOR. MANAGEMENT IS REQUIRED TO REPLACE DOOR FRAMES. REAR DOOR MUST BE RODENT PROOFED.SERIOUS VIOLATION:7-38-020 HOOO077342-16

      • Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR LEAKING MECHANICAL DRAIN STOPPER INSIDE THE THIRD COMPARTMENT SINK,EMPLOYEE ARE USING A CLOTH TO SEAL COMPARTMENT SINK. FAUCET IS LEAKING AT 3 COMPARTMENT SINK, REPAIR IT.

      • All employees shall be required to use effective hair restraints to confine hair. ANYONE ENTERING IN KITCHEN MUST WEAR HAIR RESTRAINTS,INCLUDED DIETITIAN. ALSO ALL EMPLOYEE'S PERSONAL ITEMS MUST BE REMOVED FROM KITCHEN AND PREP AREA(COAT,HANDBAG,SHOES,SLIPPER ETC.)PROVIDE A LOCKER ROOM.

NO CITY OF CHICAGO FOOD SANITATION CERTIFICATES WERE POSTED ON THE PREMISES AT TIME OF INSPECTION. INSTRUCTED MANAGEMENT THAT A CERTIFIED FOOD MANAGER MUST BE PRESENT WITH THEIR ORIGINAL CERTIFICATE POSTED ON PREMISES WHILE POTENTIALLY HAZARDOUS FOODS ARE PREPARED OR SERVED. PREMISES SERVES HOT BREAKFAST, LUNCH AND DINNER TO RESIDENTS MONDAY THRU SUNDAY. SERIOUS VIOLATION 7-38-012.

OBSERVED CIGARETTE BUTTS ON THE GROUND OUTSIDE OF THE REAR EXIT DOOR TO THE KITCHEN. MANAGEMENT INSTRUCTED TO CLEAN AND MAINTAIN THIS AREA.
