The grill station lowboy cooler has an 61 f ambient air temperature. Cooler tagged held for inspection until repaired and inspected by the Chicago Department of Public Health. Fax a tag removal request to (312) 746-4240 when the equipment is ready for re-inspection.

      • All cold food shall be stored at a temperature of 40F or less. Approximately 10 pounds of cold cuts, cheeses were found at 58.8 f internal temperature. Approximately $30.00 worth of product was discarded at this time.

      • All food should be properly protected from contamination during storage, preparation, display, service, and transportation. The cold buffet was being stocked with product this morning and the sneeze guards are cracked and broken glass. The product was discarded and the building engineer removed the broken glass guards. Buffet tagged held for inspection until repaired and inspected by the Chicago Department of Public Health. Fax a tag removal request to (312) 746-4240 when the equipment is ready for re-inspection.

      • A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Per inspection # 565431 dated 7/15/2011: The 2-compartment sink still leaks, repair the faucet so it no longer leaks.

      • All employees must wear garments that are clean and of washable character and nature. All employees shall be required to use effective hair restraints to confine hair. AN EMPLOYEE WAS OBSERVED COOKING HAMBURGERS IN HIS STREET CLOTHES AND WITHOUT A HAIR RESRAINT, WORKER WAS INSTRUCTED TO DON A WORK JACKET AND HAIRNET.

      • Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Leaky faucet at front 3-comp sink must be fixed.
