ALSO, SEVERAL EXIT DOORS TO THIS FOOD ESTABLISHMENT ARE NOT INSECT/RODENT PROOFED.THERE IS A 1/2" GAP ALONG THE BOTTOM OF THE DOOR TO THE 2ND FLOOR. MUST MAKE DOORS TIGHT FITTING ALL ACROSS THE BOTTOMS TO PREVENT PEST ENTRY.RODENT MICE DROPPINGS FOUND IN THE 2ND FLOOR FOOD PREP AREA,DRY STORAGE ROOM.PEST CONTROL LOG BOOK MUST BE COMPLETE WITH ALL REQUIRED DOCUMENTS INCLUDE A MAP LOCATOR INDICATING ALL PEST CONTROL SERVICES PROVIDED.
