    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • Outer openings not protected or rodent proofed. Front entry door has 1/4 inch gap across bottom and top of door. Instructed to rodent proof door to prevent rodent and insect entry. Pest control logbook not updated or maintained. Instructed to provided all required documents and have available during inspection. Serious citation issued 7-38-020.
    • 34. FLOORS: CONSTRUCTED PER CODE, CLEANED, GOOD REPAIR, COVING INSTALLED, DUST-LESS CLEANING METHODS USED
      • Floor not maintained through-out premises. Instructed to detail clean floor and replace any missing floor tile.
    • 35. WALLS, CEILINGS, ATTACHED EQUIPMENT CONSTRUCTED PER CODE: GOOD REPAIR, SURFACES CLEAN AND DUST-LESS CLEANING METHODS
      • Ceiling not maintained through-out premises. Instructed to replace damaged ceiling tile.

      • No hot running water at utility sink and water pressure low at all sinks on premises. Instructed that premises must provided hot running water at utility sink and increase water pressure at all sinks.
    • 41. PREMISES MAINTAINED FREE OF LITTER, UNNECESSARY ARTICLES, CLEANING EQUIPMENT PROPERLY STORED
      • Unecessary articles stored in basement and rear storage area. Instructed to remove all unsed articles and elevate supplies 6 inches or higher and away from wall.
    • 70. NO SMOKING REGULATIONS
      • Observed male employees behind counter smoking cigarette and proceed to disposal in cup. Also, observed numerous cigarette butts in basement. Instructed that smoking is not allowed inside establishment. Serious citation issued 7-32-020(a).
