    • 06. Are proper protocols being followed in regards to client care?
      • 1. Diapering procedures were observed in the green room. Staff did not wipe down the safety belt. Staff report the belt gets too damp when sprayed.July 30, 2025: New disinfectant was purchased. The contact time is 10 minutes which would be too long. Please find a disinfectant with shorter contact time.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Blue and teal room: Chemicals such as hand sanitizer was being stored in the pull-out drawers that are easily accessible to children. The chemicals were removed during the inspection.- Please ensure chemicals are stored in areas that are not accessible to the children. Consider adding safety locks to the drawers where chemicals will be stored.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. Diapering procedures were observed in the green room. Staff did not wipe down the safety belt. Staff report the belt gets too damp when sprayed.- Please use an alternative sanitizer to ensure all surfaces including the belt are sanitized.
    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. Blue room: There is water damage on the ceiling. The facility is currently in the process of having the leak addressed,
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • 1. Violet room: The handwashing station located in the classroom had an empty paper towel dispenser. The operator placed paper towels in the dispenser during the inspection.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. Violet room: The dinosaur carpet is stained and in disrepair. The corner is starting to fray.- Please remove/replace the carpet.2. Yellow room: The small plastic couch is ripped in some areas and the foam is exposed.- Please replace the couch.3. The small gym mat pieces were ripped exposing the foam layer. The items were discarded during the inspection.4. The washing machine located in the sanitizer room: There is build-up of dirt and debris.- Please wipe down the washing machine.5. Blue room: The step stool for the diapering station had build-up of debris and it was sticky to the touch. Staff wiped down the step stool during inspection.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • 1. The water tables located in the blue room and yellow room have valves that can be a potential hazard. Children can stick their finger inside the valve.- Please make modification to ensure children cannot stick their finger inside the valve. There may be a missing component like a rubber hose that came with the water table.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • 1. The bedding/cots for green room and orange room are stored incorrectly. The blankets are touching one another.- Please arrange the cots and bedding to ensure they do not touch.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • 1. The violet room + storage room: Areas around the classroom are cluttered and disorganized. - There were items above the lockers that were partially hanging off and can potentially fall onto a child.- Items in the storage area were stored on the ground and cluttered.2. Teal room: There were food debris from the sensory tables that has not been cleaned up.Please clean and organize the spaces. Consider adding storage bins with labels to organize the space. Staff will be able to find items easier.It is important to keep the space clutter-free and organized. It will be easier to spot any pest activity (ex. mice activity) and also prevent any injuries (ex. items falling off, tripping hazards etc.)
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • 1. Teal room: The toilet training seat was being stored on top of a garbage bin making the bin inaccessible. Please either remove the toilet training seat completely from the room or install a hook to store the training seat in a sanitary manner.

      • May Inspection Observation:The covering for the gym mat is ripped exposing the foam layer. Please repair/replace covering to provide cleanable surface.August 13, 2024 update: Covering for the gym mat has been removed for repair.

      • Gloves were not changed before dressing the child. Please discard dirty diaper and used gloves at the same time. Don new gloves to dress the child.

      • The covering for the gym mat is ripped exposing the foam layer. Please repair/replace covering to provide cleanable surface.
