Inspection summary
30th Street Group Home has been inspected 8 times by Interior Health Authority since December 6, 2019. The most recent inspection, on March 27, 2026, recorded 1 infraction.
1 of 8 inspections on record found no infractions at all. In the past 12 months there were 2 inspections with 4 infractions recorded. All recorded findings relate to upkeep, equipment or paperwork rather than direct health risks.
Recent inspections have recorded fewer infractions than earlier ones, so the record is improving.
What these findings mean
Health authorities word their findings differently, so we group them by what they mean for the people in care. Higher-risk findings are the ones that could directly make someone sick, and they matter more here because young children, older adults and people who are unwell are more vulnerable to infection.
Permits, records and postingLower risk
Operating permits, required signage, written procedures and logs. These findings are administrative: the documentation was missing or out of date at the time of inspection.
Recorded 15 times across 6 separate inspections, most recently on February 24, 2026. For example:
5 - Are incidents and notifications reported and records retained as required?
Equipment and utensilsLower risk
Equipment has to be made of cleanable materials, kept in good repair and stored properly. These findings are mostly about upkeep, like a cracked surface or worn part. They are generally lower risk but show how well the place is maintained.
Recorded 2 times across 2 separate inspections, most recently on August 25, 2022. For example:
1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
Other findingsLower risk
Findings that do not fit the categories above, such as follow-ups that confirm an earlier violation was corrected, or requirements specific to this type of facility.
Recorded 4 times across 4 separate inspections, most recently on March 27, 2026. For example:
1 - Does the facility provide food services which meet nutritional needs and preferences for persons in care?

- Observation(s): During a review of weekly menus, it was observed that not every snack contained at least two food groups as described in Canada's Food Guide. Submit by April 23, 2026, evidence that within all weekly menus, each snack contains at least two food groups as described in Canada's Food Guide.
- R5.1D - Provide for each day, at least 2 nutritious snacks with at least 2 food groups described in Canada's Food Guide; 62( 2 )(b)

- R4.5 - Are incidents and notifications reported and records retained as required?
- Observation(s): Substantiated Finding - RCR s. 77(2)(a)(b)(c)(d): - Allegation of a repeated failure to submit incident reports to Community Living British Columbia (CLBC) by the Licensee. Evidence reviewed details that following each allegation of harmful actions/reportable incidents, which occurred between May and September of 2025, the Licensee did not immediately notify the following parties: * Parent or representative, or contact person, of the person in care. * Medical practitioner or nurse practitioner responsible for the care of the person in care. * Medical health officer, in the form and in the manner required by the medical health officer. * Funding program - CLBC. Without immediately notifying the required parties when PICs are involved in reportable incidents, there is risk of the Licensee to be in non compliance with legislative standards, and increased risk to the health, safety, and dignity of PICs. The Licensee submitted a corrective action plan, on February 27, 2026, identifying mitigating measures implemented to ensure the required parties are notified when PICs are involved in reportable incidents.

- R3.1 - Do the manager and employees meet the employment requirements and are they assessed regularly for performance?
- Observation(s): Substantiated Finding - RCR s. 68(4): - Allegation that an employee attempted to administer a PRN (as needed) pain medication to a person in care (PIC) without administering the PIC's regularly scheduled pain medication as directed by Medication Safety and Advisory Committee (MSAC) policy. Evidence reviewed details that the employee assigned to administer 0700 h medications did not follow MSAC policy by not administering the regularly scheduled 0700 h medications for this PIC. The accused staff member details they did not review the PIC's Medication Administration Records (MAR) to determine whether the regularly scheduled pain medication had been administered prior to telephoning the program manager to receive permission to administer the PRN medication. Instruction was provided by the program manager to not administer the PRN medication. The procedure for "omitted medications" was also not followed as directed by MSAC policy. When employees do not follow MSAC policy there is increased risk of medication error incidents for persons in care. The Licensee submitted a corrective action plan, on February 27, 2026, identifying mitigating measures implemented to ensure employees adhere to MSAC policy. Unsubstantiated Finding - RCR s. 68(4): - Allegation that an employee administered a PRN medication, to a PIC for behaviours, prior to utilizing de-escalation or redirection methods, at exactly 0700 h which was the start of the employee's shift. Evidence reviewed details that the employee arrived on shift prior to 0700 h, utilized de-escalation and redirection methods to assist the PIC, and followed MSAC policy. The PRN medication for behaviours was subsequently administered at 0700 h.
