      • A broken wall and damaged baseboard were observed in the out-of-school care room. Additionally, an open hole was noted on a wall in the toddler room.Conduct a thorough check of all classrooms for open holes and ensure they are properly repaired.

      • A non-functional heavy window roller blind was stored near the kinder room window where children were sleeping. The operator removed the blind from the area and later sent photos of it installed.
    • 06. Are proper protocols being followed in regards to client care?
      • No chlorine was detected in the sanitizer bottle in the preschool washroom diapering area. The operator indicated that diapering doesn’t occur often in that washroom. The operator made a new bleach sanitizer solution which was tested at >100 ppm.

      • -A severely damaged and moldy ceiling tile was observed in the hallway. -Several ceiling tiles throughout the facility had water stains.-One ceiling tile was missing in the toddler room.Replace any affected ceiling tiles. All ceilings, walls, and floors must be easily cleanable and in good repair.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • The antiseptic wipes in the first aid kit were expired. The operator provided photos of valid antiseptic wipes.

      • -A slow drain was noted at the hand sink in the OSC room.-A slow drain was noted at one of the two sinks in the preschool/kindergarten washroom.Please fix the drainage issues.

      • Two diapering mats were stored on the floor beneath the toilets in the preschool/kindergarten washroom. This creates an unsanitary condition that poses a contamination risk. The operator relocated the diapering mats to a shelf.
