      • Operator has obtained three spore test kits by Eikon. One was completed and mailed in and two more kits are available for use. Please follow up with Eikon for a copy of spore test results.

      • Spore test not completed. Owner has single-use disposable clamps for back-up. Owner contacted Eikon to order a spore test which should arrive in the mail in a week's time.

      • High level disinfectants used for reprocessing reusable equipment are not verified using test strips, and records are not kept to track and monitor reuse period. Owner ordered test strips to implement this procedure or may opt to use the steam sterilizer on all reusable equipment instead.

    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • 70% isopropyl alochol is used for antisepsis of skin. The bottle in use did not contain an NPN number. A second bottle does contain the NPN number. Antiseptic products must have either a DIN or an NPN issued by Health Canada. Please use only the product with a DIN or NPN.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility documentation is not kept with the client service documentation and consent forms. Load numbers and verified internal chemical indicator results for all critical equipment that is reprocessed in a personal services facility and used in a personal service must be kept with that client’s records. Sterility assurance documentation from all critical equipment purchased as sterile that is used in a personal service must be kept with that client’s records.

      • High level disinfectants used for reprocessing reusable equipment are not verified using test strips, and records are not kept to track and monitor reuse period.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • A spore test is not conducted each month that sterilization occurs. Ensure one spore test is run during the month of reprocessing and sterilization.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • Manual for sterilizer and energy emitting equipment was not readily available at the time of inspection. Please have all manuals readily available for energy emitting equipment, steam sterilizer, and any device that comes with a manual.
