There was no log sheet being maintained in the facility to show records that reused CS20 disinfectant was tested daily.**Ensure that a record of daily test of the reused CS20 solution is being noted on the log sheet.**A copy of the HLD log sheet was emailed to operator2. The CS20 in the reprocessing tray did not pass the viability test. Solution was discarded.**Ensure reused high-level disinfectant is tested daily to ensure optimum concentration for adequate disinfection of semi-critical tools3.
