      • *High level disinfectant (HDL5) testing records were reviewed, and it was observed the disinfectant was being reused for more than 14 days. Change practice. Ensure when product is decanted from the bulk supply it is counted as day 1 and disinfectant can only be reused for 14 days. *Verification/testing of the disinfectant concentration within the 14 days reuse period was not completed every day the disinfectant was reused. Reviewed with operator proper testing method. Ensure the disinfectant is tested to confirm adequate concentration every day it is used with in the 14 day reused period. Please send this office a copy of the testing logs for review.

      • *No high-level disinfectant logs are being maintained. Logs need to be maintained when high level disinfectant is use. See example provided. No test strips were available to test the concentration high-level disinfectant. Please get test strips
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • *Reprocessing area is in a living space, please post a sign indicating sink is business use only.
