    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Pest activity was observed under the staff table located at the back of the facility.Please ensure that the mouse droppings are carefully cleaned by spraying them with a 1:9 chlorine solution and allowing it to sit for five minutes. Use appropriate personal protective equipment (PPE), including gloves and a mask, to minimize exposure to potential contaminants. Locate any holes and gaps and fill them with steel wool, then seal them to prevent the entry of unwanted pests. Please provide the most recent pest control report to the undersigned PHI for evaluation.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • The following areas were observed to be caked with dirt and covered in food debris, including but not limited to:- Under all shelving units in the facility- Underneath prep tablesPlease clean and sanitize the indicated areas.

    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • The maximum temperature of the high-temperature mechanical dishwasher was measured at the dish level using a maximum-registering thermometer to be 66.8°C (first reading), 70.0°C (second reading), 70.9°C (third reading). The inspector informed the staff/manager that the dishwasher must be tested daily at the dish level using a maximum-registering thermometer. A thermometer was available on site but it was not maximum-registering. The built-in thermometer was also providing false readings and tend to be not accurate. Thus, please ensure to obtain a maximum-registering thermometer and use it to test the dishwasher at the dish level daily.**OBTAIN A MAXIMUM-REGISTERING THERMOMETER.
