      • Observation (CORRECTED DURING INSPECTION): Observed opened bucket of pickles out at room temperature in the dry storage area, 2 boxes of onion rings received roughly 40 minutes ago out at room temperature.
      • Corrective Action(s): Store all products in cooler able to hold an air ambient temperature of 4C or lower. When receiving products, ensure to place all potentially hazardous foods in cooler immediately.

      • Observation: Two to three house flies observed on site. Gold fly traps are available throughout the back. Manager mentioned that the fly population usually increases in the summer months and that flies usually enter through the drive through window. Pest control visits the premise frequently. Pest control reports were verified during time of inspection.
      • Corrective Action(s): Contact pest control company for assessment, control and/or treatment options if fly population increases. Contact the health inspector if the population of flies increases and for any consultation purposes.

      • Observation (CORRECTED DURING INSPECTION): No paper towels available at front hand washing station.
      • Corrective Action(s): Paper towel roll placed near front hand washing station at time of inspection. A paper towel dispenser must be in place--refer to violation code 308. Ensure all hand washing station are supplied with hot and cold running water, liquid hand soap, and single use paper towels at all times to facilitate proper hand washing.

      • Observation: Multiple flies observed throughout premises.
      • Corrective Action(s): Ensure that measures are taken to effectively reduce pest populations as flies may potentially contaminate food and/or food contact surfaces. Limit potential pest attractants (e.g. open food, spill, grease) and follow pest control operator recommendations. Date to be corrected by: July 18, 2019.

      • Corrective Action(s): Ensure the above-noted areas are properly cleaned. Food debris and/or grease may potentially attract pest activity and may potentially contaminate food and/or food contact surfaces. Date to be corrected by: Today.

      • Observation: The following was observed:
      • 1. Beverage dispenser located closest to take out window was observed with a drain tube leading directly into a bin. An accumulation of liquid was observed in the bin. It was stated that drain pipe is leaking so tube was redirected into bin temporarily.
      • 2. Lower shelf bracket is missing from front under counter cooler.
      • 3. No paper towel dispenser is available for front hand washing station.
      • Corrective Action(s): 1. Ensure above-noted pipe is repaired or replaced to eliminate leak. Have drain tube properly routed and remove catch basin/bin. Standing liquid may attract pest activity. Date to be corrected by: July 2, 2019.
      • 2. Ensure bracket is in place at above-noted shelf and remove cardboard that is currently being used to support shelf. Date to be corrected by: July 2, 2019.
      • 3. Ensure a paper towel dispenser is in place for the front service area hand washing station. Date to be corrected by: July 2, 2019

      • Observation (CORRECTED DURING INSPECTION): A single use paper cup was observed in a bag of sugar in the front area. This is a repeat violation.
      • Corrective Action(s): Paper cup was discarded. Ensure that single use cups/utensils are not being used as scoops as they are not intended for reuse and may degrade over time. Scoops must be stored in a manner that prevents potential contamination of food items (e.g. scoop stored with handle outside of food product). Opened sugar bag should be stored in a food grade, pest proof container with a tight fitting lid. A reusable, purpose-built scoop with handle may be used granted the handle is not stored in contact with food product.

      • Observation (CORRECTED DURING INSPECTION): Quat sanitizer buckets were measured at approximately 50ppm sanitizer concentration.
      • Corrective Action(s): Front bucket refilled with fresh solution at 200ppm during inspection. Ensure dishwashing area bucket is refilled prior to next use, and that staff change the solution more frequently to maintain strength at 200ppm.

      • Observation (CORRECTED DURING INSPECTION): Back door to access corridor was left open during inspection. This has been observed and discussed at past inspections.
      • Corrective Action(s): Closed during inspection. Ensure staff keep this closed when not in active use. A tightly-fitting screen door must be installed if staff wish to leave this open, as the corridor is not finished or maintained to the same standard as the restaurant kitchen.
