- Operators are not following manufacturer's instructions to disinfect foot baths. The product being used requires a 10-minute contact time before being wiped down to kill household bacteria and viruses.

- Nail drill bits intended for multi-used observed to be used and unsanitary stored with clean drill bits around the facility.Please ensure used drill bits are stored in a manner that prevents cross contamination and is separated and identifiable from clean equipment.

- Used nail drill bits that are unsanitary and have buildup of debris observed at multiple working stations.Debris on the drill bits should be removed with a brush after every single nail service, then clean in warm soapy water, then in a disinfectant solution. If debris isn’t removed then disinfectant won’t be able to work as it will be blocked by the debris.

- Open waxing containers observed having open candy wrappers and a large accumulation of dust in and around the wax. Ensure clean products are being protected against contamination.

- Repeat violation The waxing bed is visibly soiled, hard to reach areas around the facility are soiled or have a buildup of dust and other debris. Working station, drawers observed to be unsanitary, including equipment such as desk lights, tool storage baskets, and other nail service equipment. Previous ViolationWaxing bed table blanket is not being changed in between clientele. Please ensure not only the paper lining is being changed but also the bedding. - Operators are not following proper waxing procedures, operators shall not double dip applications methods into wax.

- Sharps are not being disposed of right away into sharps container, sharps are being stored in open soap dish by handwashing sink, operator mentions once full then they are placed in sharps container.

- Work stations, are not being maintained or cleaned. Observed dust buildup of nails, on edges, drawers, of all work stations. Ensure all surfaces of work stations are being cleaned, and disinfected. Some equipment, sinks, storage carts and chairs also observed to be unsanitary. Ensure all surfaces are being cleaned routinely. Ensure surfaces are being disinfected.

- 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
- 1. Staff were observed putting on new gloves, but not observed washing hands prior to glove use. No hand washing was observed before taking a new client.2. a small scrub brush was observed in the washroom on the sink and staff indicated it was for client use. Brushes should not be shared between clients without proper cleaning and disinfection. REQUIRED:Ensure staff are washing hands after finishing with one client and before performing a service on another client. Hands must be washed before and after glove use. Remove the brush from use in the washroom, and ensure staff and clients are not sharing unclean tools.
- 15. Is reusable equipment handled and stored properly (Critical)?
- Dirty tools including two foot graters were left laying on the ground in the service area after clients had left. Dirty equipment was also left dry in the sink to be cleaned at a later time. Proper storage until they can be cleaned will help prevent them from contaminating other surfaces or people and prevent them from drying out before being washed. REQUIRED: Dirty tools should be immediately removed from the client service area after use and placed in a sealed container intended for dirty tools only, in a solution.

- No detergent present for cleaning equipment prior to disinfecting. Equipment was being washed with hand soap under running water, then rinsed. REQUIRED: Obtain a proper detergent for cleaning equipment and ensure the equipment is scrubbed in a detergent and water solution. Scrub items beneath the surface of the soap solution to prevent splashing into eyes, or clean surfaces.
- 20. Is reusable equipment disinfected properly?
- 1. the container of CS20 was not large enough to submerge the foot graters. Foot graters were only being sprayed with disinfectant. 2. staff indicated the tools were submerged for 5-10 minutes in CS20. CS20 requires 20 minutes in the solution to disinfect tools. 3. Chemicals used for disinfecting the foot baths was in an unlabeled spray bottle. Contents was unknown. Ensure all bottles of chemicals are properly labeled with their contents. 4. One bottle of test strips were expired in 2022. These were discarded. 5. High-level disinfectant records were incomplete. Product was being tested daily and recorded, but information on the disinfectant and the test strips was missing.

- Drawers and shelves in the nails stations were found to be covered with dust/debris and in an unsanitary condition. REQIRED: Clean all drawers and shelves in stations and store tools/equipment in clean conditions.
- 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
- 1. Staff indicated that clients will wash hands in the reprocessing sink when it is busy. 2. Dirty tools were being stored in the sink before washing. There was no designated space for dirty tools to be stored to keep them separate. REQUIRED: 1. Ensure all clients are only washing hands in the washroom hand sinks. Clients are not to access the reprocessing area of the facility. 2. Provide a container with lid to store dirty items until they can be washed and disinfected.
