      • Peeling duct tape was observed on the diaper changing mat located in the toddler 1 room, potentially trapping contaminants. The mat was also not equipped with safety straps.The operator promptly replaced the diaper changing mat with one in good repair and equipped with safety straps.

      • The toddler 2 room was missing clean and dirty baskets for cloth towels.Please ensure the room is equipped with distinguishable clean and dirty containers for cloth towels.

      • 1) The black highchair and its straps were soiled in stains and food debris.Please clean and sanitize the black highchair.2) The handwash station faucet handle located in the toddler 1 room was loose and in disrepair.Please repair or replace the faucet.3) The grey rug located in the toddler 1 room were soiled in stains and debris.Please deep clean the area rug.

      • Chlorine sanitizing solution measured at 1000ppm was being used throughout the facility without a rinsing step. The approved concentration of chlorine sanitizer solution is 100-200ppm, with 1000ppm being used during outbreaks.The operator was instructed to reduce the concentration to 100–200 ppm, ensure a minimum contact time of one minute, and allow surfaces to air dry.

      • 1. Infant Room: One of the covers/seats on the high infant chair was in disrepair. Ensure that the seat is repaired or replaced so it can be properly cleaned.2. Toddler room: The diapering mattress was ripped. Please repair or replace it to ensure proper cleaning and sanitation.

      • 1. Preschool room, the garbage bin was missing a self-closing mechanism. Repair or replace the lid with one that includes a self-closing feature to promote better hygiene.2. Activity room, there was a spider web on the ceiling and behind the sensory table. Clean it and maintain the area to promote cleanliness and a safe environment.

      • Preschool area, a dirty toy storage box with toys inside was observed. Staff cleaned the box on-site. Ensure that all equipment is kept clean and sanitary at all times.

      • Toddler room, children's water bottles were stacked on top of each other. Staff should ensure that bottles are not touching and clean them if the mouths are contaminated. This violation was corrected on-site.
