- The written procedures were updated at the time of inspection. Ensure written procedures are updated as needed. Personal service workers must know where these written procedures are kept. Written procedures must be presented to a PHI upon request.
- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- Re-iterated the requirement to ensure that sterility documentation for critical instruments used during a personal service is being kept with the client's records.

- There was an opened package of filler. Once the package is opened, sterile equipment is no longer considered to be sterile and must be immediately discarded after use. Discussed proper protocols.

- Bacteriostatic saline is a preserved saline that is not permitted for injection services. Discard and discontinue use.
