- *** Neuromodulators (Botox and Dysport) were not used according to the manufacturer's instructions for use (MIFU). Neuromodulators were:- reconstituted with bacteriostatic saline- not labeled with the time of reconstitution. December 16 and 17 dates were found.- Syringes were pre-drawn more than 1 hour prior to their use.Corrective Actions taken:- Ensure neuromodulators are reconstituted with preservative free saline, labeled with the date and time of reconstitution, and used within 24 hours post-reconstitution. - Refrain from pre-drawing syringes. Syringes must be used within 1 hour after being drawn to
