Professional pest control company contracted weekly, last visit was on May 8th. Continue to work with professional pest control company to rid facility of cockroaches.

      • Main Kitchen:1) Insert containers of cooked chicken were stored at room temp and measured 21C (destroyed). PHI received different responses on how long and why items were stored out. Please maintain written time/temp logs on cooling, cooking as necessary. Store perishable foods at or below 4C, or at or above 60C.Kafe 45:1) Cooler prep table measured 9C. Please adjust temp to 4C.

      • A few cockroaches were seen in a trap in the main kitchen and in the cupboards near the dishwasher in the 2 east serving kitchen.ACTIONS REQUIRED:-Please thoroughly clean and disinfect the cupboards in the 2 east serving kitchen and around the grease trap in the main kitchen dishwashing area.-Continue to conduct more frequent thorough cleaning and disinfecting in the main kitchen and 2 east serving kitchen.-Service the grease trap in 2 east serving kitchen.-Continue to follow the advice of the pest control company to treat and eradicate the cockroach infestation.

      • Dead cockroaches were seen in traps in the main kitchen dishwashing area and in 2 east serving kitchen. The majority of the dead cockroaches seen on 2 east were in the cupboards around the dishwasher & grease trap.ACTIONS REQUIRED:-Please thoroughly clean and disinfect the entire main kitchen and all serving kitchens.-Service the grease trap in 2 east serving kitchen.-Continue to follow the advice of the pest control company to treat and eradicate the cockroach infestation. Please also provide documentation of the work completed.

The flooring was lifting by the dishwasher in the 7th floor serving kitchen.-Please repair the flooring.3. There was some staining on the ceiling tiles above the 3rd floor south serving kitchen.-Please confirm that there are no recent water leaks and replace any water damaged ceiling tiles.4. There was a gap where silicone was missing on the counter behind the hand wash sink in the bistro area.-Please seal the gap to prevent water damage.5. The mop sink in the janitor room was leaking.-Please repair the leak.

      • 1. There was some food debris on the cold holding transport carts that were stored in the main walk in cooler.-Please clean the cold holding carts on a frequent basis.2. There was a buildup of juice spilt on the shelves under the juice machines and fruit flies in this area.-Please thoroughly clean the juice machines and shelves.
