      • Written procedures for cleansing of skin and accidental cut and puncture were not completed. Please complete these written procedures.

      • There was no low level disinfectant on-site for disinfecting equipment and surfaces that contact intact skin and hair. Operator had bleach, but could not locate low level disinfectant.

      • Wall behind reprocessing sink was not smooth, non-porous and easy to clean. Refinish wall.
