- Single-use equipment was not being discarded after each use. Unclean and used nail files and buffers were observed at the nail station.- Ensure single use nail files are discarded after each use.

Pre-empt CS20 and test strip to test the efficacy of the solution was not available. - Please ensure to use a test strip to verify the disinfectant is maintaining the minimum effective concentration. - Records must be kept for the date of tests (verification of minimum effective concentration), test strip results (pass/fail), dates when disinfectant was changed (for reuse period), and initials of the personal services worker who performed the activity.2. The facility was using PreEmpt RTU solution that was expired as of November 2025.- Operator is to replace solution immediately.
