      • Single-use equipment was not being discarded after each use. Unclean and used nail files and buffers were observed at the nail station.- Ensure single use nail files are discarded after each use.
    • 20. Is reusable equipment disinfected properly?
      • 1. Pre-empt CS20 and test strip to test the efficacy of the solution was not available. - Please ensure to use a test strip to verify the disinfectant is maintaining the minimum effective concentration. - Records must be kept for the date of tests (verification of minimum effective concentration), test strip results (pass/fail), dates when disinfectant was changed (for reuse period), and initials of the personal services worker who performed the activity.2. The facility was using PreEmpt RTU solution that was expired as of November 2025.- Operator is to replace solution immediately.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Accumulation of dust and debris was noted on the nail station, client chairs, and underneath foot pedals of pedicure chairs.- Please thoroughly clean the surfaces and make sure it is cleaned on a frequent basis.
