- The sanitizer solution measured a QUAT residual of greater than 400 ppm. The solution was diluted to 200 ppm QUAT residual.

- There was a bowl being used as a scoop and stored in the yeast culture in the walk-in cooler. Use a scoop with a handle and store in a way that prevents the contamination of bulk foods.

- -Some of the facility-made breads were not labeled at all. The staff confirmed that no labeling is done, and the breads are packaged in the food grade paper bags to sell to the customers.
