    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Pest control services were conducted at the facility and mall on the following dates:October 24, 2025: Pest control report noted 26 cockroaches in cabinet traps located in the front area.October 27, 2025: During a follow-up inspection, no cockroaches were observed on glue boards at the time of inspection. However, the pest control report identified 13 nymphs and 2 adult cockroaches in both front and back areas. Pest activity previously noted had not yet been resolved. The pest control operator inspected all areas of concern, replaced and added glue boards, and applied bait gel into cracks and crevices in the front and back areas.October 30, 2025: Pest control report indicated 7 cockroaches were found in the facility during treatment.Required Actions:Continue submitting pest control reports to an Executive Officer of Alberta Health Services. Reports must outline all control measures taken and the facility’s pest management plan.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • A bulky silicone finish was observed in the holes and gaps beneath the cabinets.-Ensure that all gaps and crevices beneath the cabinets are sealed with a smooth, fine silicone finish.

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • Quat sanitizer in the red sanitizer bucket and the Sani sink of the 3-compartment sink was less than 150 ppm. * New sanitizer solutions were prepared for the red sanitizer bucket and the Sani sink which was tested at 200 ppm.-Ensure that the Quat sanitizer is maintained at 200 ppm and is changed every 2 hours.
    • 07. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Critical)?
      • The plastic food wrap was stored right beneath the paper towel dispenser.* The operator moved the plastic food wrap. -Ensure that articles related food/food is protected from contamination during storage or handling.
    • 08. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Non-Critical)?
      • 1)Personal items such as purses and coats were stored on top of beverage containers.-Ensure that the articles not associated with or required for the operation or maintenance of food areas are stored separately to prevent contamination.2) Water from the dipper well was dripping onto the ice scoop which was stored in a moist bucket located inside the dipper well cabinet.-Ensure that the ice scoops are stored in a clean container and are protected from contamination.
    • 15. Is the facility free of a pest infestation?
      • A significant cockroach infestation was observed within the food establishment. Both live and dead cockroaches, representing various life stages, were found on pest control glue boards. Live cockroaches were noted on glue traps located under the dipper well cabinet, beneath the three-compartment sink, and behind the ice machine. No written records of pest control measures were available on-site at the time of inspection. According to the pest control reports that were eventually provided by the mall, there has been a marked increase in cockroach activity from May 2025 through to October 17, 2025, the date of the most recent pest control treatment.-Hire a pest control management company to inspect, treat, and eradicate the food premises of all pests, namely cockroaches. Follow the guidance provided by the pest control management company. A report from the pest control company management must be provided to an Executive Officer of Alberta Health Services outlining the control measures and future pest management plan.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • 1) Open pipes and floor drains were visible within the cabinets located beneath the dipper well, the three-compartment sink, and behind the ice machine.2) The caulking between the three-compartment sink and the adjacent food counter was deteriorating and coming off. -Seal the gaps and holes and replace the caulking. 3) Raw wood was exposed on the storage shelves.-Refinish/repaint the shelves so that it can be cleaned and sanitized.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • The cabinet located beneath the dipper well was visibly water damaged. The wooden floor of the cabinet showed signs of expansion due to prolonged exposure to moisture.- Remove/replace the water damaged cabinet and find out the cause for the water damage.
