      • Operator alerted the PHI that the chlorine inlet tube to the dish washer had broken off and that a repair man has been called to come and fix it.In the meantime, you can use the dish washer to wash and rinse, but set up the sink (next to the dish washer) with a hot water and bleach solution (of 100 ppm chlorine concentration minimum) - and only use this sink for clean dish sanitizing.Then put the dishes back in the rack to air dry.Do this process until the dish washer is fixed.

      • Need to clean both sides of the deep fryer, there is oil and food debris accumulating.Operator reports that this will be cleaned up as part of his range hood professional cleaning that is scheduled for Oct 15, 2024.Jan 28, 2025 - the identified area for cleaning had been done, but it needs to be done again - this should be a part of a weekly cleaning schedule.

      • In the outside shed, there is a chest freezer with a wooden lid that has damaged or missing hinges.Please repair or replace the hinges on this chest freezer so that the lid stays in place properly.

      • Initial testing of sanitizer in the final rinse of the dish washer was 0 ppm Chlorine.Operator re-primed machine and cleaned out some of the scale deposits in the line and the machine was achieving 50 ppm chlorine in the final rinse.Operator reported that he would de-scale the lines and the fitting into the dish washer that night. This should improve the flow of sanitizer into the dish washer during the rinse cycle.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • The hand wash sink is stocked, but was not accessible ( there was a garbage can and mop bucket in the way ).Operator moved the obstacles away from in front of the hand wash sink.

      • Previously - The porcelain tile floors of the kitchen area were found to be cracked, in a few places. Properly constructed floors facilitate proper cleaning and sanitizing. Please ensure the appropriate repairs are made.A hole in the wall was noted on the right hand side by the back door on top of the counter.Requested to close the hole.Nov 30, 2023 - the cracked porcelain floor tiles will be repaired by Dec 31, 2023 - as agreed to by the operator. Cracked floor tile areas included - in front of the deep fryer, next to the prep table cooler and in front of the dish washer.

      • Tomato sauce, butter were left open in the fridge.Flour bowl was open in the fridge.Onion rings and fries packages were open in the freezer.Always ensure food is covered
