- 03. Are clients at this facility being protected from hazards (Critical)?
- Spray bottle with 1000ppm chlorine was not labelled. Corrected during the inspection.
- 04. Are clients at this facility being protected from hazards (Non-Critical)?
- Prepared meal and fruits with high moisture content was served directly on the table to some infant in the infant room. The wooden table was previously cleaned with 500-1000ppm chlorine solution, then wiped down with soap and water, raising concerns about chemical residue mixing with ready to eat food. Please use appropriate tableware to serve food.

- Faucet at the handwashing sink next to the paper towel dispenser in the junior pre-school washroom was loose. Please repair or replace faucet.Doorknob in the toddler room was loose. Please repair or replace door handle.
- 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
- Cracked Lego block building base was observed in pre-kinder room. Cracked base was discarded by Operator during the inspection.
- 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
- The weather stripping at the back door of the OSC was in disrepair, light was observed through a gap in the right corner. Replace damaged weather stripping and seal all openings to prevent entry of pests and vermin.
- 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
- There were no paper towel dispensers in both bathrooms in OSC. Install paper towel dispensers in the bathrooms to prevent grey water from splashing on paper towels used for drying clean hands.
- 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
- Significant accumulation of pigeon droppings observed on the facility exterior front and side windows, the exterior landing and the facility signage. Bird deterrent spikes installed seems ineffective as live pigeons were observed behind the spikes. White fecal matter is widespread on window surfaces and can be observed from the road highway. Required Action: Thoroughly clean and disinfect, pigeon roosting area, all window surfaces and surrounding areas where fecal matter has accumulated. Consult a licensed pest control operator to assess and implement an effective bird control solution.

- 04. Are clients at this facility being protected from hazards (Non-Critical)?
- A door leading to the kitchen had no safety lock/knob. The staff present installed a safety knob on the door to prevent kitchen access by the kids.
- 06. Are proper protocols being followed in regards to client care?
- The diapering mat in the baby room had no safety belt at the time of inspection. The belt observed in the room was not functioning properly.Please ensure the mat is supplied with a functional safety belt.
- 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
- The paper towel dispenser in the prekinder washroom had no paper towel. The staff replaced the paper towel during inspection. Please ensure handwashing sinks are always suppled with paper towel.
- 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
- 1. The diapering mat in the Toddler room was torn at the edge. The porous material/foam was exposed. Please replace the mat and ensure all mats in the facility are always in good repair, non-absorbent, smooth and easy to clean. 2. The surface of a green chair in the Prekinder room was peeling off. Please repair or replace the damaged chair. Ensure the surface is smooth and easy to clean.
- 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
- Sleeping mats in the baby room are stored on top of each other without separation. The mats were not being disinfected daily after use. The PHI discussed proper disinfection of mats before storage or creating a separation between each mat. The staff indicated that mats will be properly disinfected after use and before storage. Please include this routine on your daily cleaning checklist and ensure the staff follow the procedure.
