      • Observation(s): The facility has had some physical changes inside and outdoors. New fencing, extended cement poured area, bathroom changes with removal of doors. Licensing was not notified of the changes. One staff person did not have a training certificate posted as well as certificates were photocopies of the original. Original or notarized copies are accepted.
