      • No written procedures on the following tasks:-the cleansing of skin and mucous membrane-application of antiseptic products -post-service care for personal services that involve puncturing the skin or mucous membrane- client care in the event of an accidental skin cut or puncturePlease provide written procedures on these tasks.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Microneedling needles and derma paneling blades were not documented on the client information sheet.Document needles and blades lot number and expiry date on the client information sheet.
    • 20. Is reusable equipment disinfected properly?
      • Hydrafacial machine probes were with 70% alcohol only. Required to use a disinfectant with a drug identification number (DIN).
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1) Used roller waxes were found on the heating element. Discarded the roller waxes. Roller waxes are one time use only. Please discard after use on one client. 2) Zensa Lidocaine cream was expired since March 2024.Discarded the lidocaine.3) Alcohol pads were expired since August 2025.Provide new alcohol pads.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Dr. Pen A6 was found in the facility.Dr. Pen A6 was banned by Health Canada. Please stop using it.
