      • **Pending violation**Vents throughout the facility had dust accumulated.- Clean the vents.Update from November 26,2025:The wooden child gate in the infants' room had rough edges posing a risk of splinters.-Sand and seal the gate.

    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • 1) In infant room diaper change table station is not in immediate vicinity of handwashing sink. The handwashing sink is in a gated area, and the staff has to open the gate each time to go wash hands after diapering. - Move the diaper table to the be within the gated area near the handwashing sink or keep the child gate open at all times.2) Handwashing sink in toddler 1 was blocked with bibs.- The handwashing sinks always have to be accessible.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • 1) Handwashing sink was blocked with children's bibs and the staff started washing hands on top of the bib's.- The operator was educated that they cannot wash hands on top of other stuff and that bibs cannot be put in the handwashing sink. The sink must be available at all times for handwashing and must be used only for hand hygiene. The operator moved the bibs to a bucket they use to store dirty bibs and cleaned the handwashing sink.2) No paper towel was provided at the handwashing sink in preschool 1 and toddler 2 room. - The operator filled the paper towels during the inspection. Handwashing sinks should be supplied with paper towels, soap and hot and cold running water at all times.

      • 1) Vents throughout the facility had dust accumulated.- Clean the vents. 2) In the OSC room the toy bins had tape, and it had dust and debris accumulated. - Remove the tape.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • The yoga mats in the preschool 3 were all worn off and had dust accumulated.- Replace the mats.

    • 16. Does the social care facility have adequate lighting?
      • Dark in the downstairs bathroom; increase the lighting as it is a high task area.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Repair the door Insteps and weather stripping as discussed.Mop and buckets were stored adjacent to the children's toilets.Repair the downstairs bathroom exhaust fan.Repair the gaping baseboards in the downstairs bathroom.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Reiterated the same infraction as below; Please resolve this situation. Infant room (Room 1)- diaper change table station is not in immediate vicinity of handwash station. Staff must leave gated area to handwash. In interum, advised staff to ensure handsanitizer is provided in the immediate area and to ensure handwashing is done before and after glove use. Diapering station will have to be moved to be next to handwash station for immediate access without gate barrier

    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • The following were noted in the Toddler Room (Room 2):1. Converted floor vents- wooden box cover is warm to touch )@30 deg C) with top mounted metal vent cover measuring 52 deg C on surface. Obtain assessment from furnace technician to determine if boxed structure meets building code. 2. Wall mounted space heater- provide information from manufacturer that this unit meets safety accreditation for use in Canada. 3. Electrical cords for wall mounted heater are duct taped to wall. Cover for thermostat box is also duct taped to wall. Ensure the above items are covered/concealed properly to prevent access by attendees. 4. At time of inspection, an odor was detected in this room. Technician onsite was assessing the box covered heaters to determine if source of smell was from here. Identify and remove source of odor.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • The following items were corrected during inspection:1. Toddler Room (Room 3)- -small objects were in toy shelving area that are small enough for choking hard (small plastic pieces). Removed from toy bins. 2. Preschool Room (Room 4)-plastic container storing small paint jars was cracked. Removed by staff. 3. Preschool Room (Room 5)-Bag containing blue salt had small debris inside. -Moon flour- prepared with untreated flour. -Bingo dapper stored in cabinet is not approved for childcare use.All items removed.
    • 06. Are proper protocols being followed in regards to client care?
      • Debris observed under diapering change mats Ensure after use, mats (including undersides) and surfaces of diapering table are cleaned and disinfected properly
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kits- those containing alcohol wipes did not have expiry dates. One designated bag in the Before and After care room did not have bandaids contained in the designated kit. Operator has already preordered new first aid kits for the centre. Facility will no longer be using alcohol wipes for first aid applications.

      • 1. Additional cleaning of the following areas were noted:- Preschool Room (rm 4) ceiling fan had dust on surfaces of the fan base. Please clean fan surfaces to remove dust. - Toddler Rm (Rm 3) - Table-top had a white adhesive film on surface that was lifting. Please remove film on surface to provide smooth clean surface for cleaning & sanitizing -Infant room (Rm 1)- the cupboard below handsink had stains and debris on inside bottom surfaces. The upper shelving cabinets above handsink had adhesive liners that were worn and lifting. The floor vent in the play area also had buildup on vent cover. -Preschool Room (rm 6)- some of the chairs had residue buildup on the back side of back supports. The Stage area had gaps between stage and wall where loose items had collected in void spaces. 2. Utility room- many items are stored at floor level. Please organize and keep items off the floor to allow for ease of sweeping and pest monitoring
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • Preschool (Room 3)- washroom for toddlers had mop and mopbucket stored on the floor next to the toilets. Please provide a separate area for storing cleaning equipment .
    • 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
      • Observed small amount of droppings on floor in utility room by catch trap. Ensure droppings are removed and monitor for new activity.
