      • The knob on the teacher’s cabinet located in Room 1 was loose and fell off during use, exposing the sharp end of the screw.Please repair the knob.

      • 1) Bingo markers not safe for children were found throughout the facility.The operator was informed about acceptable markers for children, labelled with an "AP" stamp or labelled as non-toxic with an age range. The operator removed the bingo markers. Please sort through all craft materials and remove anything potentially hazardous to children.2) A broken shell was observed in Toddler Room 1, creating sharp edges.The operator promptly removed the shell. Please monitor the basket of shells and remove any in disrepair to eliminate any potential hazards.3) No safety locks were installed on the teacher's cabinet with personal belongings.The operator installed child safety locks on the cabinets.

      • 1) A woven straw basket was used in Room 1 and 2. Woven straw baskets are absorbent and have grooves and crevices trapping contaminants.The operator promptly removed the woven baskets and replaced them with smooth, easily cleanable plastic bins.2) A shaggy rug was used in Room 2. The long fibres from the rug can trap contaminants make it difficult to thoroughly clean.The operator promptly replaced the rug with one with short fibres and easier to deep clean.3) The leather couches located in Room 1, and the Room 3 were worn with peeling surfaces making it difficult to effectively clean.The operator promptly removed the couches from the facility and have informed the inspector of plans to replace them.4) Wooden toy blocks located in the Kindergarten Room were unfinished with rough surfaces that can absorb and trap contaminants.The operator promptly removed the unfinished wooden blocks.

      • 1) The wall located beside the entry door to Room 1 was in disrepair, exposing the dry wall.Please repair and refinish the wall to ensure the surfaces are smooth, easily cleanable, and impervious to contaminants.2) The knob on the teacher’s cabinet located in Room 1 was loose and fell off during use, exposing the sharp end of the screw.Please repair the knob.

      • Water infiltration and flooding were observed in the basement, specifically near the stairwell, the right OSC classroom, and within the furnace room.Please ensure the following: - Retain a restoration company to assess and remediate the source of the water.- Provide AHS with a report that outlines the following:• Identification and description of all issues discovered• Affected areas documented and clearly outlined• Corrective actions taken, including any completed repairs

      • A paper towel dispenser was empty in an OOSC washroom. Some paper towels were stored on top of a mirror. - Refill the paper towel dispenser. Store paper towels adequately protected from contamination.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Used cloths were sitting at sinks in most classrooms. - Discard/place wiping cloths in a laundry bin after use.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Uncleanable straw hats were being used in OOSC program. - Remove the hats since they can not be cleaned/ disinfected or laundered.
