      • 1) The top right corner of the bulletin board in the infant 2 room located on the partition wall was in disrepair, leaving sharp edges.Please repair the corner to prevent accidental punctures or cuts.2) The tamper proof mechanism of the receptacle located in the toddler 1 room was in disrepair.Please install protective covers or replace the receptacle.3) The corner of the mirror located in the toddler 1 room was cracked with tape used as a temporary repair.Please replace the mirror.4) Clothing hangers located beside the play kitchen in the toddler 1 room were installed at eye level, creating a potential protrusion hazard.Please reinstall the hangers higher, to a level that is above children’s eye level.

      • The chlorine sanitizer solution located in the preschool 2 room was tested and measured at 1000ppm. 1000ppm chlorine solution is intended for use during outbreak situations.The operator was instructed on the proper preparation of a chlorine sanitizer solution at 100ppm. The existing sanitizer solution was discarded and remade, then tested and confirmed to measure 100ppm.

      • 1) The following handwash stations had cracked or peeling caulking behind the faucet:- Right side washroom located in the preschool 2 room- Left side washroom located in the preschool 1 roomPlease remove the worn caulking and reseal the joint with a new bead.2) The diaper changing mat located in the preschool 2 room was cracked and peeling, creating crevices that can trap contaminants.Please replace the diaper changing mat.

      • The play kitchen located in the toddler 1 room was dusty.Please clean and disinfect all surfaces of the play kitchen including the interior cabinets.

      • Mold and mildew were observed around the rubber gasket of the laundry machine.Please clean and disinfect the indicated area to ensure all the mold and mildew are removed.

      • Preschool 1: Some toy containers were dirty. A shelf for storing water bottles was dirty.- Clean/disinfect regularly. Debris accumulation on chairs in upper-level rooms. - Clean regularly.
