- A container of Lysol wipes and a spray bottle of an unknown mixture were stored underneath the handwash sink located in the infant room. Child safety locks were not installed, and chemicals were accessible to children.The operator was instructed to remove the Lysol wipes and the spray bottle. Please ensure chemicals are stored properly and inaccessible to children.

- Four highchairs located in the infant room had broken clips, which could prevent proper restraint of infants.Please repair or replace the highchairs.

- Spray sanitizers throughout the facility were measured at 500 ppm, which exceeds the recommended concentration of 100 ppm.The operators were instructed to remake the sanitizer spray bottles and were educated on the proper mixing ratios for sanitization, diapering, toy cleaning, and food contact surfaces.
