      • A container of Lysol wipes and a spray bottle of an unknown mixture were stored underneath the handwash sink located in the infant room. Child safety locks were not installed, and chemicals were accessible to children.The operator was instructed to remove the Lysol wipes and the spray bottle. Please ensure chemicals are stored properly and inaccessible to children.

      • Four highchairs located in the infant room had broken clips, which could prevent proper restraint of infants.Please repair or replace the highchairs.
    • 06. Are proper protocols being followed in regards to client care?
      • Spray sanitizers throughout the facility were measured at 500 ppm, which exceeds the recommended concentration of 100 ppm.The operators were instructed to remake the sanitizer spray bottles and were educated on the proper mixing ratios for sanitization, diapering, toy cleaning, and food contact surfaces.

      • 1) The black garbage can located in the preschool room had visible stains on its surface.Please clean and disinfect the garbage clan.2) Ceiling tiles throughout the daycare were water-stained, indicating a water leak.Please address the water leak, replace the damaged ceiling tiles, and continue to monitor the area for any further leaks.
