      • A roll of paper towels located beside the diaper station in the toddler's classroom was not stored in a dispenser, which could result in contamination of the entire roll.-Please provide or install a dispenser for the paper towels to ensure proper hygiene and prevent contamination.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kits missing emergency blankets and tweezers. -Please provide the identified items in all classrooms' first aid kits.

      • 1. Carpets with dirty stains and residues were observed in the toddler classroom.-Please clean the indicated carpets and ensure regular cleaning to maintain sanitary condition.2. Dirt residues were also observed under the diapering mat in the toddler classroom.-Please clean and sanitize the identified surface and ensure it is clean after each diapering procedure.

      • Some children's water bottles were observed without name tags on them in the toddler room.-Please ensure all children water bottles are labeled with their names to prevent misuse.

      • Infant room: Behind the main door there was a large hole in the wall Repair wall

      • 1. Paper towel throughout the facility handwashing stations are not protected from contamination. Paper towel must be placed in a dispenser.2. Toddler room: Handwashing sink tap is leaking. Repair sink

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Paint indicated for the use of ages 5 years and over were found in the infant and toddler rooms. Remove paints not suitable for age range of classroom.
    • 06. Are proper protocols being followed in regards to client care?
      • Diapering in toddler room: Diapering surface under the mat had an accumulation of dust and debris. Clean and disinfect all surfaces of the diapering area after each use.
    • 15. Is laundry being properly handled in a suitable manner?
      • Wet wash cloths were found throughout the facility. Wash cloths must immediately be set aside to be laundered after use.

      • Bathroom and toddler room sinks: Backsplash of sinks had buildup of soap and scum. Clean and disinfect indicated area

      • The gate at the bottom of the stairs leading to the back pay area is not secure. Ensure this is repaired or replaced.
    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. Several shelves throughout the facility, including but not limited to, the shelf in the toddler room, the shelf in the infant room, the shelf in the preschool room, are damaged, worn or have raw wood exposed. Ensure these are repaired or replaced.2. The wall in the OSC classroom by the door is in disrepair and has drywall exposed. Ensure this is repaired and painted.

      • There is a build up of debris on the floors, walls, on step stools within the washroom. Ensure these areas are cleaned.

      • The disinfectant bottles throughout the facility are not labelled with their contents. Ensure all disinfectant bottles are labelled.
    • 06. Are proper protocols being followed in regards to client care?
      • The diapering surface in the toddler room is made of a mesh material and is not smooth or easily cleanable. Replace surface so that it is smooth and easily cleanable.
    • 07. Is this facility properly constructed (Critical)?
      • The gate at the bottom of the stairs leading to the back pay area is not secure. Ensure this is repaired or replaced.

      • 1. Several shelves throughout the facility, including but not limited to, the shelf in the toddler room, the shelf in the infant room, the shelf in the preschool room, are damaged, worn or have raw wood exposed.       • 1. There is a build up of debris, dirt and grim throughout the facility, including but not limited to, on walls, hard to reach areas of the floors, shelving, tables, chairs, carpets, etc. Ensure the facility is thoroughly cleaned. 2. The facility will need to develop a detailed written cleaning plan that must include what is cleaned daily, weekly, and month, by whom, and sign off by staff.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Several toys throughout the facility have a build up of debris/grim, were broken, had raw wood exposed, soft chairs were in disrepair, wooden doll houses had insides peeling with raw wood exposed, etc. Go through all of the toys. Any toy that can be cleaned, clean and disinfect. Any broken toys discard.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • There is a build up of debris on the floors, walls, on step stools within the washroom. Ensure these areas are cleaned.
