    • 09. Is a point of service risk assessment being performed of the condition of the client and of the equipment to be used on the client prior to performing a personal service?
      • *A rusty manicure tool was found in the clean storage equipment container located at the manicure station. Remove from service. Ensure that the equipment to be used as part of the personal service is visibly clean and in good condition and repair. Tool was removed during inspection.
    • 20. Is reusable equipment disinfected properly?
      • *Debris was noted inside the barbicide disinfectant jar. Management reported that the barbicide was being changed once per week. The manufacturer’s instruction stated the disinfectant must be prepared fresh daily. Discard the contaminated barbicide and revise the practice to ensure the disinfectant is made daily. Barbicide was discarded on site. *PHI reviewed barbicide preparation instructions as staff were not measuring the barbicide concentrate ( 62.5ml of barbicide into 1L of water). Please ensure barbicide is prepared according to the manufacturer’s instruction.*Hair was noted on multiple rollers. Reclean and disinfect all the hair tools. Rollers were removed and placed into dirty tools area for cleaning/disinfection.

    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • *Evidence of reuse of single use tools, single use nails files were found in the storage with nail dust. Immediately stop reusing single use nail fails. Immediately stop reusing single use items and discard used items. Ensure single use equipment is used according to manufacturer instructions. Items discarded during inspection.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • *Disinfected mani/pedi tools were found stored in dirty containers. Nail clippings were found in the storage container. Clean out container and reclean/disinfect tools. *The drawer located at the mani station was very dirty. Nail clipping and debris were found. Clean out area and ensure salon is maintained in in a sanitary condition.
    • 20. Is reusable equipment disinfected properly?
      • *High level disinfectant (Preempt CS20) testing records were reviewed, and it was observed the disinfectant was being reused for 15 days. Change practice. ensure when product is decanted from the bulk supply it is counted as day 1. Manager corrected the date on the existing record being used. Please review with all staff members.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • *Container of wax was found open with a used wooden stick on top. Ensure wax pot is closed after use to protect wax from contamination and used stick is discarded. Review with all staff. Stick was discarded and lid was put onto the wax pot.

    • 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
      • *Written procedures in place for single sink used for hand washing and equipment decontamination. Please add contact time to procedure for disinfectant wipes. The sink should be disinfected not sanitized. Please update wording for procedures.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • *Written procedures not in place for facility. Review services being offered and ensure applicable written procedures are completed and made available onsite at all times. Template will be sent with a copy of the inspection. (e.g. Accidental skin cuts or punctures, cleansing skin etc.)*Only one sink present in facility for hand washing and reprocessing. Written procedure required for hand washing and reprocessing sink decontamination.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • *Paper towels were missing at the sink. Please ensure paper towels in a proper dispenser is available at the one compartment sink.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • *Evidence of reuse of single use items. (e.g. nail block, nail files, wooden sticks for waxing). Nail dust noted on single use items and used wooden stick sitting on top of wax container. Operator indicated manicure items are reused after its cleaned and disinfected. Discussed manufacture does not have a label claim indicating products can be reuse. Reviewed with operator AHS EPH poster of the types of single use products not allowed to be reuse. Poster will be emailed to operator. Immediately stop reusing single use items and discard used items. Ensure single use equipment is used according to manufacturer instructions. Items discarded during inspection.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • *Staff also go into resident rooms for manicures. A plastic bag being reused to transport tools. Please change storage practice. Tools should be kept in a storage container that can be cleaned and disinfected between uses.
    • 20. Is reusable equipment disinfected properly?
      • *Electric clipper blades were not being cleaned or disinfected in between clients. Only the removeable guard was being placed into barbicide. Lots of hair was noted in the electric clipper. Ensure electric clipper including blades are cleaned and disinfected between use. Discussed the use of Clippercide, as it is a cleaner and disinfectant made for electric clippers.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • *Storage area for waxing equipment was disorganized. Cotton balls were not stored in a proper container. Water bottle was present. Clean and organize area and ensure all equipment and tools are properly stored.
