- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- No sterility assurance documentation available to review. Sterility assurance documentation is required for equipment with a sterile claim (needle cartridges).Documentation must include the following: - The name of the client- The contact information of the client - The date of the service - Description of the service - The name of the staff member conducting the service Ensure the sterility assurance documentation is completed for every service and every client. These records must be maintained for at least 2 years.
- 19. Is ultrasonic cleaning of reusable equipment being performed properly and when necessary?
- The ultrasonic solution is reused for more than five days. The operator stated they reuse the solution for approximately two weeks however, the manufacturer's instructions indicate a five-day reuse period.The manufacturer's instructions for the ultrasonic solution must be followed to ensure tools and equipment are adequately cleaned prior to disinfection. Follow the manufacturer's instructions and change the solution every five days or whenever it becomes soiled.
