- Observation: The Permit To Operate does not have the correct facility name/ Chevron on it.
- Corrective Action: Obtain a replacement Permit, attach the current year decal to it and post the Permit in a conspicuous location in the facility.

- Observation: The written Food Safety Plan is not acceptable.
- Corrective Action: Provide a written Food Safety Plan on site. It must contain procedures that are described in the Food Safety Plan template that was provided to the Operator, Simm on November 13, 2924.

- Corrective Action: Repair the wall surface so that it is tight, durable, non-absorbent and free of cracks & gaps.

- Observation: Utensils are sanitized after being washed with a QUAT solution. The operator does not know what the QUAT concentration being used is. It must be atleast 200 ppm.
- Corrective Action: Sanitize utensils in a QUAT solution that is atleast 200 ppm. Use QUAT test strips to verify the concentration.

- Observation: The dry goods storage room has boxes of food stored directly on the floor. This prevents adequate cleaning of the floor and promotes the contamination of food containers.

- Observation: The janitorial sink area is damaged and dirty and cluttered with overlapping wet mops. These conditions attract pests and promote contamination of the dry goods storage room.
- Corrective Action: Clean and sanitize the janitorial sink area. Repair the wall surface around the sink so that it is smooth, free of cracks & gaps and non-absorbent.

- Observation: The floor under front of the Taylor slushy machine has accumulated sticky liquid. The accumulation is an attractant for pests.The liquid is leaking from the machine.

- Corrective Action: Ensure to use ice cubs and keep coffee creams at 4 degrees Celsius or lower.
