      • Sterility assurance documentation for sterile equipment was not retained with the clients' records. Ensure that sterility assurance documentation for all critical equipment used during services is retained with clients' forms.

      • A product used for disinfection of laser machine attachments did not have a DIN. Approved disinfectant (with a DIN) is required.

      • Manufacturer's instructions for use of cosmetic products were not being followed: a). bacteriostatic saline was being used for reconstitution, while manufacturer requires a preservative-free saline be used. Please obtain and use preservative-free saline.b). the reconstituted neuromodulator product was re-used for over 24 hours: the reconstituted Dysport product (reconstituted on May 2) was observed in the fridge.
