      • Some bottles of disinfectant spray had no expiration date. Operator was unable to confirm expiration date from bottle.- Ensure disinfectant solution expiration dates are recorded if transferring to smaller spray bottles to use.- Bottles were replaced with disinfectant wipes during the inspection.

      • Non-preservative saline expired October 1, 2025. - Ensure staff are checking expiration dates and replacing when needed.

Please ensure the sterility information of the pre-purchased sterile equipment used on the client is documented on the client's forms and available for review.

      • Dysport is reconstituted with saline with preservatives. Dysport's manufacturers instruction for use requires to use preservative free saline.Please follow the product's manufacturer's instruction while using it on clients.
