Cockroaches were also observed on a pest monitoring device located by the west walk in cooler, and in a trap in the basement food preparation space under the two compartment sink.Continue to work with your professional pest control operator towards the goal of eliminating cockroaches from the facility. Ensure that adequate pest monitoring activities are taking place. November 1, 2024:Cockroaches were observed in pest monitoring devices located in the main kitchen and in the basement. Three were observed under the three-compartment sink, and one was observed in the trap near the entrance of the main kitchen. A total of six cockroaches were observed in the sticky traps located in the basement.Regular pest control is put in place by a professional pest control company. Continue to work with the pest control company to monitor and control pest activity. - Used cleaning cloths were observed sitting on counter outside of sanitizer solution. Used cleaning cloths not in use must be submerged in sanitizer solution to kill harmful bacteria.***To reuse cleaning cloths to wipe down surfaces, set up sanitizer buckets with chlorine sanitizer solution measured at 100 ppm or quat sanitizer measured at 200 ppm and submerge the cleaning cloths in the solution when it is not in use.

- Employee personal water bottle was found in the sandwich cooler on the pizza cookline. To prevent cross-contamination, personal items should not be stored alongside commercial food products.***Operator promptly moved the water bottle to an area away from commercial food products.

- 1) Eggs were stored on the top shelf of the counter cooler located at the bar above beverages. Eggs could potentially break and contaminate the items below with harmful pathogens.***Please relocate the eggs to a bottom shelf below beverages2) A mixing bowl was observed to be used as a scoop in a bulk container of gnocchi located in the walk-in freezer. Scoops with handles prevent cross-contamination between the operator's hands and the food product.***Please remove the mixing bowl and replace it with a handled scoop.3) Boxes and containers of food were observed on the floor of the walk-in cooler. Food stored on the floor is at risk of contamination from debris tracked in by shoe soles and give easy access to pests.***Please elevate the food off the floor by 6 inches or more.

- The plexiglass front of a display case was less than 152 cm (5 feet).To protect against customer contamination (coughing, sneezing etc), the height of the display case must either be at least 183 cm (6 feet) in height, or at least 152 cm (5 feet) with at least a 31 cm (1 foot) return portion.

- The chlorine dishwasher located at the bar was tested using testing strips, which indicated a reading of 25 ppm. To ensure effective sanitation, the chlorine sanitizer must register at 100 ppm. It's important to note that chlorine sanitizer is essential for eliminating harmful pathogens.***Please service the dishwasher so it provides the concentration of 100 ppm for sanitizing dishes.

- Soap bag out of its dispenser was observed at the hand sink on the hotline. There was no dispenser observed around the hand wash sink. Hand washing supplies must be easily accessible to promote correct hand hygiene procedures.***Please install a soap dispenser or provide hand soap that is easily accessible for all employees working on the hotline.

Clean and sanitize the surfaces with frass on the walls around the laundry machine.
