      • **There was no disinfectant on site for disinfecting the reusable hydro fascial machine tips.ACTION REQUIRED:1) Purchase a low-level disinfectant for disinfecting the hydro fascial tips between clients.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • **The microneedling needle cartridges were missing sterility assurance information on the packages. The needle packages were in a zip log bag. The operator stated that they purchased them from the institute that trained them. Cartridge packages were missing manufacturer information, method of sterilization, process indicator etc. ACTION REQUIRED:1) The operator was directed to put the needle cartridges on hold until they are able to provide sterilization assurance documentation with lot numbers that matches those on the cartridges.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • An uncleanable foam pad observed on the service bed. The operator was directed to remove the foam pad and not to use it because it cannot be cleaned and disinfected.
