This form must be signed by the client prior to the service and must include pre-screening to identify contraindications, and an explanation of the risks and benefits of the procedure.2) Create a system to track the batch/lot numbers of all pre-purchased sterile, single-use equipment (e.g., microneedling cartridges, dermaplaning blades).

- **PREempt CS20 is used for disinfecting tools. The operator is diluting the CS20 prior to use. The operator stated that she soaks the tools in CS20 for at least 2 hours, then rinse and dry them before storage. ** There were no test strips for the CS20** Disinfectant logs for the CS20 were not being maintained as required- The operator stated that she changes the CS20 every week.ACTION REQUIRED1) You must cease diluting the CS20. The CS20 shall be used as it is2) Purchase test strips for the CS20. 3) Maintain daily logs for the CS20 using the template provided. Test strips shall be used to verify the concentration of the CS20 each day that it is used. The CS20 shall not be used pass a failed test or beyond the 14 days reuse period.
