    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures for preparation of skin and decontamination of dual-purpose sink.Ensure the written procedures are developed.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Roll-on wax cartridges were observed. The operator informed that she gives away the leftover wax to the client it has been used at and the client could bring the cartridge next time for use. The operator was advised that the roll-on cartridges are single use, and it is not the best practice to re-use it even if she is using it on the same client.
    • 20. Is reusable equipment disinfected properly?
      • -Non-critical equipment is not being adequately cleaned and disinfected. Ensure non-critical equipment is cleaned and disinfected using, at a minimum, a low-level disinfectant.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Three spray bottles were not labeled.Please ensure only cosmetic products with a detailed label are available for use.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • - Handwashing is occurring at a sink used for equipment reprocessing. Written procedure for a dual-purpose sink was reviewed with the operator.
