      • Missing written procedures for the following: 1) Cleansing of the Skin.2) Applying Antiseptic 3) Aftercare instructions4) Accidental Cuts & Punctures.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Expired alcohol wipes (2024). **Discard and obtain new alcohol wipes. ***DISCARDED AND 70 PERCENT ALCOHOL SPRAY AVAILABLE.
    • 20. Is reusable equipment disinfected properly?
      • Preempt RTU wipes and CS 20 were expired. **Discard. At a minimum use a low level disinfectant for non critical equipment. Highly recommend to use intermediate level disinfectant such as Preempt RTU for dermplaning handle. ***CORRECTED DURING INSPECTION. CAVICIDE WIPES WERE AVAILABLE TO DISINFECTANT ITEMS.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • A few packages of pre-sterilized needles were expired (2024). **Discard. ***CORRECTED DURING THE INSPECTION.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • A few expired inks were observed at the time of the inspection (Oct 2025). Operator noted that she keeps them on display for color reference for customers or to practice on fake skin. **Discard or label inks with "practice" or "display" only or place inks elsewhere for practice use.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • 1) Missing signage for laser use. **Include signage for laser service stating it is a laser, the classification, and indicate danger of the device. 2) Missing manuals onsite for energy emitting machines. **Send link or have manuals onsite.
