- July 9, 2026: the mouse droppings were still present in the cupboard next to the dishwasher. Staff did not clean the right cupboard. July 2, 2026: A few mouse droppings were found in the cupboard that contains the chemicals next to the dishwasher. Operator was instructed on how to properly clean and sanitize the affected area. Place pest control monitoring devices inside this cupboard to monitor for any future pest activity.Complete the AHS Pest Control Checklist once a month and keep these records on site for review at future inspections.

- No approved sanitizing solution was on-site at the time of inspection. Operator was instructed to obtain an approved sanitizing solution immediately and to have it ready for the scheduled re-inspection.

- The paper towel dispenser at the kitchen hand sink was empty. The operator filled the paper towel dispenser at the time of inspection.

- The two sliding glass door coolers in the kitchen were not turned on at the time of inspection, and therefore the internal temperature of the coolers could not be tested. Operator was advised to turn both coolers on the day before the scheduled re-inspection and to make sure they are running at 4C or colder.

- Probe-style thermometers were not available at the time of inspection. Ensure probe-style thermometers are available for monitoring cooking, hot-holding and cooling temperatures of foods.
