      • Holes observed along plumbing lines under several sinks and toilets in the facility including under the baby room handwashing sink, preschool 1 bathroom, preschool 2 bathroom, preschool 2 room handwashing sink. Please seal these holes as cockroaches can harbour in them.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Unlabeled spray bottle observed in preschool 2 room.2. Mop bucket with cleaning solution inside was stored in the toddler bathroom - this needs to be inaccessible by children.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Elevated equipment (slide with a fall height of 27.5") was observed in the toddler room without proper surfacing in place. Slide has to be removed or have proper protective surfacing 6ft around the platform of the slide.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Hand sink within preschool room: The protective case was missing from the power unit under the sink in the preschool room, wires dangling and accessible. The sink is a non-touch system with a power box below.The director found a plastic container and used duct tape to secure on.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Paper towel dispenser was empty next to sink in diapering area used by toddler/baby room clients.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. The walls, doors, high touch surfaces were visibly dirty throughout the daycare especially the washrooms.Deep cleaning with warm soapy water needed to scrub down the walls, doors, trim etc throughout the daycare and then routine maintenance. 2. Damaged and stained baseboard next to toilet in the preschool room Remove and replace with a new baseboard. Recommend vinyl baseboards adjacent to toilets. 3. The door between the preschool and younger preschool was rough to the touch/ paint crumbling. Repair or replace door so that it is smooth and easy to clean.

      • Torn blue and white diaper change pad on floor next to a toilet. Replace with one that is smooth and easy to cleanEnsure to hang it up out of reach from children
