      • The freezer was observed with large ice buildup on the inside. The outside of the cooler was also observed with ice along the door where customers would retrieve food items. You can see staining along the bottom of this cooler where ice was observed (freeze, thaw). CORRECTEDThe hot holding unit was observed with the side missing; advised the glass broke. Cardboard covered in foil has been used to replace the glass. Recommend waiting until it has been repaired before using again. Referral to fire.       • Walk in cooler was > 4 C (my thermometer read 8.2 C and the facilities thermometer read 9.0 C).Produce cooler was >4 C.Temperatures are not being verified daily and recorded. I recommend verifying all of the refrigeration equipment temperatures 1-2 times each day and recording. If a deviation happens a corrective action needs to be taken.
    • 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
      • 3 sink method was discussed with staff. Wash, rinse (to remove soap bubbles and any food particles), sanitize (for 2 minutes if using a bleach solution), then air dry.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • The handwash sink was blocked at the time of inspection. Staff advised that they use one of the other sinks however these sinks are not intended for handwashing. Please unblock this sink and get into the habit of using it for handwashing.
    • 18. Are general nuisance conditions, other than those noted in other violations, being prevented at this food facility (Non-Critical)?
      • Food boxes were observed on the floor up and down several isles. Foods must be stored up off the floor and extra boxes stored on shelving.
    • 19. Does the facility have a valid permit and does the facility operate in compliance with the conditions of that permit, or is the facility awaiting approval prior to operation?
      • A food handling permit application has not been received.
