    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Notification of business and services were provided in person during the inspection.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures were not established for:- skin cleansing of mucous membranes.- decontamination of a sink used for both hand washing and instrument reprocessing.- accidental skin cuts and puncturesCorrective Actions:- Please ensure written procedures are established for skin cleansing, decontamination of dual purpose sink, and when there is an accidental skin cut or puncture.- AHS written procedure templates in Vietnamese were left onsite, and will be finalized during the re-inspection.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Single use disposable items such as buffing blocks, emery files, toe separators appeared to be reused between clients. Visibly contaminated single use items were present throughout the facility, including storage trolleys and nail stations.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Metal drill bits were stored in a visibly contaminated state at workstations.Corrective Actions:- Ensure all previously used reusable metal equipment is taken to the reprocessing area to be cleaned and disinfected after each client service.
    • 20. Is reusable equipment disinfected properly?
      • Equipment was not adequately disinfected.Preempt RTU, an intermediate level disinfectant was expired since 09/2024, and used to spray on equipment.Corrective Actions:- Acquire a high level disinfectant and appropriate test strips.- Disinfect reusable equipment with high level disinfectant according to the manufacturer's instructions for use (pre-clean, 20 minute immersion, rinse and dry after, 14 day reuse)- Test the high level disinfectant solution each day it is used with appropriate test strips, according to the manufacturer's instructions for use.- Document daily test results in a logsheet.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Cosmetic products were decanted into smaller containers, including pump bottles. These containers were not labeled.Corrective Actions:- Ensure all decanted products are appropriately labeled with a product identifier, and expiry dates (if applicable).
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • General sanitation around client service areas, mainly the nail stations was unsatisfactory.There was buildup of nail dust and debris on the tables, chairs and drawers at opening time, before any clients have arrived.Corrective Actions:- Ensure all client contact surfaces are cleaned and disinfected between each client service.- Ensure all other environmental surfaces are cleaned at a frequency to ensure the facility is maintained in a clean and sanitary condition.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • The same sink was used for reprocessing equipment and washing hands. No decontamination procedures were established to prevent cross contamination between those activities.Corrective Action:- Establish procedures to decontaminate the reprocessing area when switching tasks (hand washing and reprocessing)
