      • Dented can of sliced mushrooms stored on the shelves.Canned foods should not be kept once they are damaged or dented to avoid the growth of foodborne botulism and its toxins. Inspect canned foods regularly and prior to use and discard if damaged/dented. The operator discarded the canned sliced mushrooms at the time of the inspection.

      • Observed a staff member washing their hands without soap. Proper hand washing must follow these steps to remove contamination from the hands: a. wet the hands with warm waterb. lather hands with soap c. rinse with warm water d. dry hands with disposable paper towelThe staff member was informed to wash their hands again with soap at the time of the inspection.
