- Facility-specific written procedures were completed during the inspection.
- 11. Is post-service care provided by personal service workers following any personal service or accident where a client's skin or mucous membrane is cut or punctured during a personal service, or when energy-emitting equipment is used?
- Clients were not provided with written care instructions for microneedling services. Develop written care instructions for microneedling services and provide this office with a copy. Ensure clients are provided with verbal and written care instructions following a microneedling service.
- 13. Is single-use or uncleanable equipment being used as intended (Critical)?
- Comedone loops with "piercing tips" are considered to be critical equipment that must be discarded after each use or sterilized using steam as its sterilant. The tools were removed from the facility at the time of inspection and will no longer be used. If desired, obtain comedone tools that do not have "piercing tips"
- 20. Is reusable equipment disinfected properly?
- 1. The barbicide did not contain a DIN. Obtain barbicide with a DIN for hair-related tools.2. Obtain 2 bottles of indicator test strips. If using PreEmpt CS20, buy AHP7 indicator test strips (50 count). If using PreEmpt HLD5, buy AHP2 indicator test strips (100 count). 3. High-level disinfectant logs were not being maintained. Templates were provided. Ensure HLD logs are being kept for each high-level disinfectant tray used. Keep at least a 2-year history of high-level disinfectant (HLD) logs available on-site for future inspections to prove proper logs are being maintained.4. The 1L bottle of PreEmpt CS20 had a spray nozzle. Remove the spray nozzle. Semi-critical tools are required to be fully immersed, not sprayed.
- 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
- Sterility assurance documentation was missing for the blades used for microblading services. The blades were discarded at the time of inspection. Sterility assurance documentation must be available for all equipment purchased as sterile including the following information: -lot number that matches the sterility assurance documentation -manufacturer or company name and contact information. When this info is only found on the box/container, keep the box/container for your records -expiry dates, determined by the manufacturer, based on shortest projected useful life-method of sterilization.
- 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
- The manufacturer's instructions for use (MIFU) was not available for the energy-emitting equipment (i.e. SharpLight Rapid-DPC) found on-site. The MIFU must be kept in an accessible place for reference by a personal services worker at all times. Obtain the MIFU for all energy-emitting equipment and provide this office with a copy.

Various eyelash-related cosmetic products were missing labelling requirements. -Sky Glue: missing instructions for use-GA Lash adhesive remover. 10g.: missing list of ingredients, instructions for use-BL Lashes Remover for eyelash extensions: missing instructions for use-BL Lashes lash primer for eyelash extensions: missing instructions for use2. Waxing pots were not covered. Install covers to prevent contamination of the bulk supply.3. Henna cosmetic products used for eyebrows was missing instructions for use.
