    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • November 21, 2025:Cockroaches were found on numerous pest monitoring traps in the facility. Live cockroaches were observed outside of traps in the space between the grease interceptor and the wall. Cockroach frass (droppings) were observed on some wall and ceiling surfaces. February 24, 2025:7 adult and nymph cockroaches were observed in pest monitoring devices in the facility. September 26, 2024:4 adult cockroaches were observed in sticky traps around the kitchen. 1 roaming adult cockroach was observed under the dishwasher. The facility does have an active pest control program in place and traps/bait were observed throughout the facility.***Please send 3 months of pest control reports to the undersigned PHI to evaluate the frequency of treatment and activity levels.October 22nd, 2024: Cockroach activity was still observed on sticky traps throughout the back kitchen. Pest reports have not been received.November 5th, 2024: 3 cockroaches were observed on a monitoring glue board in the back kitchen. Pest control frequency has increased, and reports have been requested. Please email the reports to the undersigned PHI.

      • Mouse droppings were found on floors in several areas in the facility. Additionally, mouse droppings were found in one cupboard.Safely clean up mouse droppings. Monitor for further activity. Ensure adequate pest control is taking place in the facility.

      • There was a gap at the base of the south entry door.Replace the weather stripping or otherwise ensure the gap at the base of the south door is sealed to help prevent pest entry.

      • Several areas in the facility require cleaning, including the wall beside the ovens, the vent cover above the ovens, spaces between the grease interceptor and walls, and ceilings with cockroach frass. Clean these areas.

Covers and sneeze guards protect the food from external contamination.***Install a sneeze guard that adequately covers the pastries or cover pastries with food grade covers such as cake covers or individually saran wrap each itemNovember 6th, 2024: Glass had been ordered but when it arrived it was too small. A new piece of glass has been ordered. Until the glass arrived and is installed, please wrap each pastry individually or relocate the pastries.

If five or fewer are working, at least one manager must have approved training. All others must have at least basic food safety knowledge.

      • Concentration of quat sanitizer was too potent, testing at 400 ppm. Correct concentration of quat sanitizer should be 200 ppm. Excessively high concentrations of quat sanitizer can be harmful when ingested and irritate the skin.***Operator was instructed on how to properly mix and accurately test quat sanitizer solution. Operator was informed to test the quat sanitizer mixture every time it is made

      • The concentration of quat based food contact surface sanitizer in the wiping cloth bucket was undetectable. Solution was remade and then concentration measured 200 ppm. Ensure that solution is changed out often enough so that the concentration of quat sanitizer is maintained at the acceptable level (200 ppm).
    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • Wiping cloths were not being stored in sanitizer solution between uses.Ensue that reusable wiping cloths are being stored in sanitizer solution between uses.
    • 10. Are food temperatures during storage and processing being controlled to maximize food safety?
      • Staff were not checking food temperatures for reheating or hot holding.Ensure that temperatures are checked during reheating to ensure that the product reaches 74 degrees Celsius or higher rapidly. Ensure that hot holding temperatures are checked to ensure that 60 degrees Celsius or greater is being maintained.
