    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Please submit a copy of filled Pest Control Check list at the end of November 30, 2023 and submit it to undersigned PHI.**Pest Control check list template emailed to operator for usage on November 14, 2023. Ensure it is completed monthly and keep the records in a binder. Submit a photo once you have completed November 2023 pest control check list.

    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • No chlorine test strips or Quats sanitizer test strips to verify chlorine sanitizer strength at least 100 ppm. This is a repeat violation. It was brought up from previous inspection on Jan. 25, 2022. Submit a photo once chlorine test strips ordered and available at the facility. Facility using Mr. Clean as surface disinfectant. However, no Quats sanitizer to check Quats sanitizer at least 200 ppm. Switch to bleach water sanitizer at least 100 PPM and purchase chlorine sanitizer to verify the strength.

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • Operator switched to bleach water sanitizer and it was measured at 100 ppm ~ 200 ppm (minimum 100 PPM). Operator was using Mr. Clean surface disinfectant, however, Quats sanitizer strips measurement was 0 ppm. Quats based sanitizer needs to be at least 200 ppm. No Quats sanitizer strips at the facility.

      • Paper towels closed to be empty. Operator will purchase more paper towels. Operator took some paper towels from activity room paper towel dispensing unit at the time of inspeciton.
