      • No written procedures for skin cleansing prior to services and accidental cuts were available.

      • A re-usable hand towel was used at the hand sink. Please ensure that a single-use or paper towels are provided.

      • No records of testing of high-level disinfectant were available, and no testing of disinfectant has been conducted as required.

      • Laser Tattoo removal Model No J-200 machine had a wire detached - reported that the machine is not being used. Please repair or remove.

      • Wave length gel was used against manufacturer's instructions (expired in August 2023). Please obtain a new product.
